Why Fever Makes You Shiver

You're already running a fever, your skin is warm to the touch, and yet you're wrapped in a blanket shivering like you're standing in the cold. It feels contradictory — you're hot, so why is your body acting like it's freezing? The answer is that your internal thermostat just moved the target, and your actual temperature hasn't caught up yet.

The hypothalamus sets a temperature target

The hypothalamus normally keeps body temperature regulated around roughly 98.6°F (37°C) by comparing your actual temperature against that internal target and triggering heat-generating or heat-releasing responses to close the gap.

Infection resets the target higher

In response to infection, immune signaling molecules called pyrogens act on the hypothalamus and push that internal target upward — to, say, 102°F instead of 98.6°F. The moment that happens, your actual body temperature is suddenly below the new target, even though it hasn't changed at all yet.

Shivering is the body chasing its own new target

From the hypothalamus's perspective, you are now too cold relative to the new set-point, so it triggers the same heat-generating responses it would during real cold exposure — muscle shivering to generate heat through rapid contraction, and blood vessel constriction in the skin to reduce heat loss, which is also why skin can look pale during this phase.

Why the fever eventually breaks with sweating

Once the infection signal fades and the hypothalamus's target drops back toward normal, your actual body temperature is now above the new, lower target — flipping the response to sweating and blood vessel dilation to release heat, which is the "breaking" of a fever that often leaves someone drenched in sweat as their temperature comes back down.
