Every organ in your body has a dedicated waste-clearance system except the brain, which was assumed for decades to just not have one. It turns out it does — it just only runs the pipes at night, and mostly during the deepest stage of sleep.

A plumbing system discovered less than 15 years ago

The glymphatic system, first described in 2012, is a network that uses cerebrospinal fluid to flush metabolic waste out of brain tissue — functioning something like the lymphatic system the rest of the body already has, but built around the brain's unique structure instead. It moves fluid through spaces surrounding blood vessels, washing waste products out of the tissue that produced them.

Why it needs you to be asleep to work well

During deep, slow-wave sleep, the space between brain cells expands by a substantial margin compared to waking hours, which dramatically increases how much cerebrospinal fluid can flow through and how efficiently it clears waste. This expansion doesn't happen anywhere near as effectively while you're awake, which is a big part of why sleep — deep sleep specifically — appears to be functionally necessary for this clearance process, not just restful in a general sense.

What's actually being cleared

Among the waste products cleared through this system is beta-amyloid, a protein that accumulates abnormally in the brains of people with Alzheimer's disease. This is one of the reasons chronic sleep deprivation is an area of active research interest in neurodegenerative disease — impaired glymphatic clearance over years could plausibly contribute to the kind of protein buildup seen in these conditions, though the full causal picture in humans is still being worked out.

Deep sleep isn't the only overnight process running on its own schedule — breathing mechanics shift across sleep stages too, which is a reminder that a lot of what makes sleep restorative is happening in stages most people never think about.
