There's no obvious trigger, no clear purpose, and no reliable off switch — just a sudden spasm and a sharp little "hic" that interrupts a sentence mid-word. Hiccups are one of the strangest reflexes the body has, and one of the least well explained.

A two-part spasm, not one

A hiccup is actually a coordinated misfire between two structures: the diaphragm suddenly contracts, pulling in a sharp breath, and almost simultaneously the glottis (the opening between your vocal cords) snaps shut, cutting that breath off abruptly. The sound is the air hitting a closed door it wasn't expecting. This whole sequence is driven by a reflex arc running through the phrenic and vagus nerves to a poorly understood coordination point in the brainstem.

A theory that reaches back to gills

One of the more interesting explanations for why this reflex exists at all points to evolutionary history: the neural circuitry involved resembles the reflex that controls gill ventilation in amphibians and larval stages of some aquatic animals, and a similar pattern shows up in mammalian fetal development. The theory suggests hiccups may be a kind of vestigial echo of gill-breathing control circuitry, left over from a much earlier point in the evolutionary chain, that never fully disappeared as lungs took over.

Why they're so hard to predict or stop

Because the reflex is coordinated in the brainstem rather than under direct conscious control, you can't simply decide to stop mid-hiccup the way you can hold your breath. Most hiccup bouts resolve on their own within minutes regardless of what's done about them, which is part of why so many different home remedies (holding your breath, being startled, drinking water a specific way) all seem to "work" — many bouts would have stopped around then anyway.

Why holding your breath sometimes actually helps

There's a real mechanism behind at least one common remedy: holding your breath raises carbon dioxide levels in the blood, which appears to help interrupt the reflex arc driving the spasm. It's not guaranteed, and it doesn't work for every bout, but it's one of the few hiccup remedies with a plausible physiological reason to actually do something rather than just coincide with the hiccups stopping on their own.
